My companies 401k is limited in the selection.
After excluding Bonds, International and Company related Funds the Large, Mid and Small Cap Index Funds are left.

What is the recommendation?
Pick 1, 2 or spread over the 3?

Depends on how much you care about your allocation between the funds and how much work you want to put in re-balancing.

Personally I'd PREFER to match the allocation of a total-stock fund like VTSAX.  Don't remember its weighting off hand but it is mostly (like 80%) large-cap, the rest split between mid and small.  However, while VTSAX re-balances within the fund for me, in your situation you'd need to do this yourself periodically to maintain the desired allocation.  That sounds like more work than I would want to deal with, so I'd probably just set an initial allocation and let it ride and split my contributions using the same percentages.  In the fullness of time things will surely get unbalanced, but I wouldn't care enough to mess with it.

I would spread it and add the international.  Each has a different volatility and growth profile so each has its place.
