I would speculate that it ended up on his neck as he panicked and tried to squirm out from under the bar. Regardless, very good read.

Reason 1,000,062 that those things are pieces of worthless shit. At least with a regular bar if you do something dumb and pin yourself, you have one last shot: dump the weight. Good luck with that on a Smith machine.
