I have read through the topics on how to fill out this form. I am a little confused on two parts of the form.

4D -Model Do I use the model number of the specific brand lower that I am using?

4H -Additional Description Do I put my name / city/ state in this box, since this is what I will have to engrave with a Individual Application?

4d yes

4h leave it blank unless it was imported then put the importer in.

Ok. I guess my finally question is were does the information go about what I am engraving? Or is that information taken from 3B?

Before making a new SBR, federal law requires that you have an approved Form1.

Federal law also requires the maker to engrave his/her/it's name/city/state on the firearm.

Federal law does not require that you list your intended engraving anywhere on the Form 1.

If you do so voluntarily, it's not unlike volunteering info when being interviewed by law enforcement: Rarely does any good come of it, and it often ends up working against you.

So engrave your SBR, but do not list that info on the Form1.
