What is the oldest Form 1/4 you have seen?

When I got a Mac10 it came with a case. In the case was a copy of a Form 4 from Feb 8th, 1983.
I did a FOIA and saw that my gun was made in 82 with 10 other guns. Sold to the first owner in 1983, Sold to the 2nd owner in 2005. Then sold to me in 2010.

It would be interesting to see some old forms, wonder how many times they have been changed/revised.

We had a Thompson come through that came from a local PD with the forms from the mid '30s in the velvet lined case.

I had a form for a savage thompson with a early 50's form.
