 Throwing shots, possible flash hider problem?

PSA 20" heavy upper on a standard PSA lower, vortex 4x16 by 50mm solid one piece mount...shooting at 50 yards

first time out, put two in an inch area and then the next two were an easy two to three inches up and left...
did the exact same thing on the next 4 rounds...ammo was hornady 75gr

has a YHM muzzle device on the front, only questions are possible problem with buffer? recoil sounded rather weak
muzzle device foot pounds to tight or to loose? problem with scope? or something else?

Once that trigger is pulled the only things that effect accuracy are barrel muzzle device and scope.... If u had failure to extract, load, than buffer maybe. Has to be loose scope or u flinching like crazy. Bad ammo maybe if it is old got wet??

Confirm that the barrel nut is correctly tightened, not getting FS strikes, and the bolt is locking back on the last short out to confirm that the rifle is correctly stroking.

for the life of me i cant get the bottom of the yhm flash hider to line up with the bottom of the barrel. Torqued down it needs to be turned between 300-270*. Used the new crush washer that came with it, the flat side of the flash hider is sitting on the 3 oclock when looking down the scope....

You do have the flat small taper end of the crush washer against the shoulder of the barrel correct?

If so you may need to try a different crush washer.Personally I still prefer peel washers,but Im old timey in alot of ways so YMMV.That said I have better installs using the peel washer and its repeatable if the FH has to be removed and reinstalled for some reason.Certainly a more permanent installation,and required if you intend to use a sliencer.

Ive still got a couple old Bushmaster/Quality Parts Corp early model Vortex FH's that used a peel washer.Those required that the peel washer be adjusted to where the FH internal shoulder was as close to the muzzle crown as possible.So thats a good example of how exact the PEEL washers can be tuned.
Ive noticed some of the manufacturers have returned to peel washers as well.Got a newer Armalite T upper that came with a peel washer installed under the A2 FH.

the crush washer is on correctly, might try the old washer that came off...this is a brand new upper, only put the 8 rounds through it

I wouldn't try reusing a crush washer.Some say its okay,but I don't think so.
Personally if you think the FH is possible the issue with your groups,I'd leave it off and try shooting again for verification.Otherwise I'd put some more rounds thru the upper,try some different ammo and clean the rig per the TM..Then contact PSA for further advice.

take FH off and check end of barrel for possible nick in the crown.

Remove the muzzle devise and scope to test the rifle. Then if it is OK install one item at a time and see what happens.

Normally a crush washer is a one time deal. peel washer may work better for aligning the muzzle devise.

What barrel twist rate?

Sometimes the Hornady 75's will stabilize in a 1:9 barrel, but not always. Stabilization may be marginal. I know you identified the ammo as "Hornady 75", but is this Hornady factory-loaded ammo or reloads using Hornady bullets?
