I opened the box and the alignment tab, the part that lines up the rail with the upper receiver, is cracked and bent in towards where the receiver would be. I've contacted the seller about getting a refund, but I'm wondering how common is this? What kind of force is required to break this part of the forend?

Looks like it was dropped.

more like battered! I would not think just dropping it even if mounted would do this since it is secured to the barrel nut with the two screws.
Heck yes I would get a refund that is useless IHMO.

I wouldn't say that's useless though I'd expect a severe discount. My brand new Alpha rail's tab doesn't come close to touching the side of the receiver. I used a one piece scope mount to hold alignment while I tightened down the lock screws.

Its looking like it was damaged in shipping. Which makes some sense. as there's no protection for the tab when its not mounted. Troy sent me a RMA to send it back for inspection.
