Hammer pin walking out

My hammer pin continues to walk out with the D clip installed. Any advice or suggestions. It is the 2 stage trigger on a mid length rifle.

how tight is the "D" clip on the hammer pin itself?
Does it fit nice and tightly in the groove on the hammer pin?
Are you sure that the "D" clip was riding in that groove?
