land owner permission slips

Does anyone have an example of a land owner permission slip that they would be interested in sharing with me? I am not quiet sure what to include on it. I will be hunting coyotes at night and baiting them. Any help would be great, thanks!!!

I don't know if your state has any particular rules but it's not even very common here in Texas to get written permission, most are just verbal agreements. Here's one that we were given that the guy just wrote on the back of his business card, it's pretty informal. I can't even read the bottom part of it.

My state just states that i have to have written landowner permission to night hunt . Their website is not very helpful, as i have been coming upon new forms i need signed by a conservation officer to bait for coyotes. Kind of a pain in the ass haha
